Required for all Scouting participants — youth, parents, and leaders.

Forms are valid for 12 months from the date of the physical.

📋 What's Required

📝 Part A — Informed Consent

Required for ALL events. Release agreement and authorization signed by the participant (or parent/guardian for youth under 18).

📝 Part B — Health History

Required for ALL events. General health information and medical history. Includes allergies, medications, and past conditions. Both sides must be completed.

🩺 Part C — Pre-Participation Physical

Required for events lasting 72+ hours. Physical exam completed and signed by a certified physician (MD, DO), nurse practitioner, or physician assistant.

⚠️ Important: "All participants" includes parents, guardians, siblings, youth, staff, and leaders attending any Scouting activity. Even if you're just driving to a campout — you need Parts A & B on file.

❓ Common Questions
Who completes Parts A & B?
The parent or guardian fills these out for youth. Adults complete their own.
Who completes Part C?
A certified physician (MD, DO), nurse practitioner, or physician assistant must perform the physical and sign Part C.
Where do I submit the completed form?
Submit completed forms to your Den Leader (Cub Scouts) or Scoutmaster (Scouts BSA). They'll keep them on file for activities.
Can I use my regular annual physical?
Yes — as long as the physician completes and signs the BSA Part C form during or after the exam. Bring the form to your appointment.

Den Filtering

When Pack 1053 is selected, additional Den options appear. Filter by specific den levels (Lions, Tigers, Wolves, etc.).

Date Range

Choose a preset or custom date range:

  • 3 Mo — Current month + 2 months ahead
  • School Yr — August through May
  • 12 Mo — Full year from current month
  • Custom — Select specific start/end months
Note: Maximum range is 48 months.

Choose how events are displayed:

  • List View — Chronological table with full event details
  • Calendar Grid — Traditional monthly calendar layout

TBD Events

Events with "TBD" dates (month only) appear at the end of each month section, sorted after dated events.
